Elevo Dynamics will only freeze your membership if you are in good standing, you have all initiation fees paid, you are current on your monthly dues, you provide at least Thirty (30) days (Four weeks) before your next billing, written notice on a Freeze Request Form (Membership Change Form) to allow Elevo Dynamics to process your request, and you qualify for one of the three (3) categories below:
–Medical Disability: You must provide Elevo Dynamics with verification from your physician stating that your medical disability will prevent you from using Elevo Dynamics facilities. The minimum term for a medical freeze is three (3) months, and the maximum is six (6) months. If the disability renders you unable to use the facility and/or the facility services for longer than (6) months, then you will be able to cancel, provided you have documentation that proves your disability is the cause of your need to cancel.
–Active Duty Military Transfer or Extended Volunteer Assignment: You must provide Elevo Dynamics with a copy of your transfer, deployment, or extended volunteer assignment orders. The minimum time for a freeze is three months. The maximum time for a freeze is 12 months. Your membership and monthly Electronic Funds Transfer (EFT) or credit card charge will automatically resume at the end of the specified time.
–Temporary Employment Transfer: You must provide Elevo Dynamics with verification from your employer, on company letterhead, that you are being temporarily transferred. The minimum term for a temporary employment transfer freeze is three (3) months, and the maximum freeze is six (6) months.
Dues During Freeze
Once Elevo Dynamics approves a membership freeze, no further dues will be collected with respect to such time. Your right to use the Elevo Dynamics facility will be frozen or suspended, and Elevo Dynamics can deny you access to the dojo during such freeze. In the event your membership is prepaid, the term of the prepaid membership shall be extended, without further dues, for the same period of the freeze. Except for military freezes greater than six (6) months but less than (12) months, your membership will be reactivated, and your monthly EFT or credit card payment will automatically resume at the end of your freeze period.
Cancellation
Policy
Our Cancellation Guidelines are as follows:
-Qualification and Notice Requirements: Elevo Dynamics will only begin review of your request to cancel your membership if you are in good standing, you have all initiation fees paid, you are current on your monthly dues, you provide at least Thirty (30) days (Four weeks) before your next billing, you provide written notice on a Cancellation Request Form (Membership Change Form) to allow Elevo Dynamics to process your request, and you qualify for one of the three (3) categories below:
-Completion of Agreement Length: Agreements are 12 months, at which point they will renew for a month-to-month membership. At the point in which an agreement is a month-to-month, you can cancel provided you are in good standing (all fees and dues paid), and you give written notice on a Cancellation Form (Member Change Form) 30 days (Four weeks) before your next billing. Request received with less than 30 days' notice can only be canceled after the next billing date (provided it meets all other qualifications).
-In a case of injury or disability: If you become injured or disabled, you shall extend the duration of the original contract at no cost to you for a period equal to the duration of the disability. You must prove such injury or disability by a doctor's certificate, which shall be enclosed with the written notice of injury or disability sent to the facility. In the case of injury, the notice will have to clearly state that the injury will prevent you from further use of the facility.
-Relocation Cancellation: If you permanently relocate (or relocate for a time longer than six months) from the address you were at when entering into an agreement with Elevo Dynamics and that address is forty (40) miles away from Elevo Dynamics or one of its franchises you may be eligible for cancellation of your agreement. You will need to have proper documentation (proof of address change: bill with your name on it, military orders, etc., or if this is for youth, proof that their primary guardian is relocating. If the youth is moving to stay with another guardian, proof would have to be presented as well. Additional acceptable forms of documentation are listed below). This proof will have to be turned in with a written request Thirty (30) days prior to your next bill date. Request received with less than 30 days' notice will be canceled after the next billing date (provided it meets all other qualifications). In the event that the facility relocates fifteen (15) miles away from the location it was at when you entered the agreement, AND this distance is greater than fifteen (15) miles from your current residential address, your membership can be canceled by written request turned in Thirty (30) days before your next payment is scheduled.
Note: This policy does not apply if you already live 40 miles or more miles from the facility when you enter into your agreement with Elevo.
Acceptable forms of supporting documentation are:
• A driver's license issued within the past 60 days,
• notarized copy of lease or mortgage agreement,
• utility bill,
• cable bill,
• military orders,
• voter's registration card,
• A paycheck with the new address
-Dues During Cancellation: Once Elevo Dynamics approves a membership cancellation, no further dues will be collected. Your right to use the Elevo Dynamics facility will be frozen or suspended, and Elevo Dynamics can deny you access to the dojo.
